Complaints Procedure

At some point in your life as an amputee you may find yourself with an issue that requires a formal complaint against a prosthetic provider or a facility.

Remember, this is YOUR RIGHT and you have options.

The first step is to determine the severity of the issue.  If it is not prosthetic related, then you may want to discuss your concerns with the prosthetic facility owner or the head of prosthetics.  If that is not possible, ie they are the subject of the complaint or the complaint involves a breach of the Canons of Ethical Conduct, then draft your complaint in writing, including as many details as possible and send it to:

In the USA

American Board for Certification in Orthotics & Prosthetics, Inc.
1650 King Street, Suite 500
Alexandria, VA 22314-2747
(703) 836-7114 Fax (703) 836-0838

If your complaint is criminal in nature, ie sexual assault, fraud or theft, you should also contact the appropriate authorities IN ADDITION to contacting the National Organization.

Addresses and contact information for other countries are being compiled.
